I assume these are not Posa mounts.
What happens is that the inside face of the thumbscrew is conical shaped and when tightening them too tight or while slightly misaligned, is that the sharp edge of the "cone" flairs out with a burr. As you try to unscrew, this flaired burr hits the edge of the mount not allowing it to unscrew far enough.
Loosening the center screw will not help you if this is the case.
Unfortunately about the only solution is force. Then completely unscrew and remove the screw and file (turn) the lip down (back to shape).
By the same token, from misalignment the dovetail block can get a burr from the screw and even though the screw may be backed off enough the burr on the dovetail will not allow the mount (s) to slide, again force and clean-up required.
