A chipped bolt lug on a 700 Remington

The other day I saw a 700 Remington bolt where one of the locking lugs had a big round chip (flake) on the contact surface. The exposed grain of the bolt head steel seem to be quiet corse. The chip was round and covered some 80% of the lug hight and was quiet deep. I would guess 0.5MM (0.02").
Is this just one off or is it more common?

I would say it is pretty unusual... I have never heard of it before... I don't understand how that could happen in normal operation and firing...

Mostlikely an impurity in the steel that is not obvious without xraying the steel.
 
My first post here. Been lurking for a short while.....so Hi to all!

Not being a gunsmith in any way, but having had some experience in heat treating I think (for whatever little that's worth) that the clue is in the observation that the exposed grain seemed quite large.
Large grain, generally, means poor/improper heat treatment and a weak end product that will be subjuct to (comparatively) easy fractureing.

Just a guess...
