Actually, Pete, it becomes a problem of the combination of die & bushing that can cause the problem. The combination of the die sizing all of the neck and not allowing the bushing to float...in conjunction with bushings that don't have enough radius on their edges...is what causes the problem...

Ditto... I agree with these guys...you should be allowing at least .002" total clearance in your chamber...in other words, a loaded round should mic no bigger than .267 across the pressure ring of the bullet... Additionally, and just as importantly, do you have the button screwed down tight...
